What IUI is
Intrauterine insemination (IUI) places washed, concentrated sperm directly into the uterus near the time of ovulation. It bypasses the cervix and shortens the distance sperm travel to the egg. IUI can be done in a natural cycle or paired with ovulation-stimulating medication (letrozole, clomiphene, or low-dose injectable FSH).
Who it's for
IUI is most effective for: mild male factor, unexplained infertility, cervical factor, and donor-sperm cycles. It's generally not recommended when both fallopian tubes are blocked, when male factor is severe, or for women over 40 with diminished reserve — those patients usually move directly to IVF.
Success rates
Per-cycle live birth rates from medicated IUI typically run 5–15% depending on age, diagnosis, and medication used. Most clinics recommend 3 IUI cycles before reassessing — cumulative success after 3 cycles for unexplained infertility is roughly 20–25%. After 3 unsuccessful cycles, the marginal benefit of a 4th cycle drops sharply and most clinics recommend moving to IVF.
Cost ranges
Self-pay IUI in the US typically runs $300–$1,000 for the procedure itself, plus $50–$200 for monitoring ultrasounds, $50–$300 for oral ovulation medications, or $500–$3,000 for injectable medications. A medicated IUI cycle with monitoring usually totals $1,500–$4,000 out of pocket. Donor sperm adds $700–$1,200 per vial.
